• WELLNESS CARE: $75 per hr | 2 hr minimum Medical Escort/ In Home IV Infusions/ In Home Wound Care/ Post Operative Care/ Medication Assistance/ Wellness Support/ Patient Advocacy Placement Assistance

• ACTIVITIES LIASON: 2.5 hr $150.00+ $10.00 per resident minimum of 5 residents | Manage state mandatory calendar and monthly newsletter call for pricing

• END OF LIFE CARE: $350 monthly | 2 in home visits a month with 1 emergency visit
